Job Description
Administer occupational therapy screening, evaluation, and treatment to clients according to your scope of practice where therapy services are being provided.
Perform initial and ongoing occupational therapy evaluations on new clients, and re-evaluations as indicated.
Document client progress in daily treatment encounter notes, weekly progress reports, recertification documentation, and discharge summaries for each client in which you provide therapy services.
Provides home health as needed. Plans and implements home-based interventions, including installation of adaptable equipment for Participant safety. Conducts required home safety visits, and other visits, as necessary for home-based interventions and observation.
Provides supervision to program aides and other staff who are implementing occupational therapy treatment and/or maintenance programs.
Demonstrates active participation in QI processes, staff training, and meetings.
Provides care to Participants in-person, via telehealth, and/or via telephone, as indicated.
Qualifications
Position of Occupational Therapist requires a graduate degree from an AOTA accredited Occupational Therapy Program or equivalent for foreign trained therapists.
Spanish fluency preferred, but not required
