This is a wonderful opportunity for someone who is passionate about serving the underserved communities of Los Angeles County. As an Outreach Specialist, you will engage with community partners and build meaningful relationships to drive referrals for myPlace Health's PACE (Program of All-Inclusive Care for the Elderly). In this role, you'll educate community partners, prospective participants, their families, and caregivers about the benefits of the PACE program. Your efforts will help frail elderly individuals live independently at home while continuing to enjoy a high quality of life.
\n What does success look like as an Outreach Specialist?Drive sales and enrollment: Coordinate outreach activities across the Greater Los Angeles Area to help achieve monthly enrollment goals through community visits and self-generated referrals.Build meaningful relationships: Partner with healthcare professionals, providers, community-based organizations, senior housing, and post-acute facilities to drive referrals for our PACE program.Collaborate for success: Work closely with our Enrollment Specialist Team to ensure seamless and successful participant enrollment.Be the face of myPlace Health: Organize and host community events, deliver engaging presentations, and attend healthcare-related events to promote our PACE program and increase visibility.Keep us organized: Track and maintain all activities in Aline CRM software, ensuring up-to-date and accurate information.Stay compliant: Follow DHCS and CMS regulations to ensure all marketing practices align with industry guidelines.Shape our growth: Develop and implement quarterly marketing plans to meet enrollment goals and collaborate with center management to develop retention strategies.Innovate: Identify opportunities to enhance referral processes and continually improve outreach activities.Stay flexible: Attend evening and weekend events as needed to engage with our community.Contribute to the big picture: Take on additional responsibilities to support the success of myPlace Health's Marketing and Outreach strategies. What would a candidate bring to the table?Experience in healthcare or enrollment sales, with a proven track record of generating business-to-business or business-to-consumer referrals.Consistent success in meeting monthly enrollment and census goals.Experience collaborating with State/County eligibility workers and other referral professionals.Proficiency in CRM software is a plus.Experience working with frail elderly individuals is highly valued.Self-motivated and independent: Able to work effectively with minimal supervision.Education: A B.S./B.A. degree in Healthcare, Business Administration, or Marketing is preferred but not required.Bilingual skills in English and a second language most commonly spoken in Los Angeles required. Preferred Languages are Spanish, Korean, Tagalog. Transportation requirements: This role involves regular travel throughout the Greater Los Angeles Area. Candidates must have a valid CA driver's license, reliable personal transportation, a good driving record, and auto insurance as required by state law.

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
