We have an immediate need for a full-time Patient Coordinator to join our Case Management team. The Patient Coordinator plays a vital role in ensuring effective communication and support between our healthcare team and our patients. Offers are contingent upon passing a background check.
\n Responsibilities The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources. Accompany patients to their benefit renewal appointments to advocate for the patient and the program.Visit patient homes and update patient care. Effectively communicate and provide information to patients, caregivers, and physicians. Understand HIPAA and maintain confidentiality.Display a warm and friendly attitude. Minimum QualificationsStrong work ethic and relationship building skills.Self-motivated with the ability to manage time appropriately.Strong communication skills inside of the office and outside.Experience with MS Office products (Outlook, Word, Excel, etc.)Minimum associate level education preferred. (High school diploma or equivalent is required.) 1 year of medical knowledge preferred.Dependable transportation 
\n$23 - $25 an hour
$100 monthly cell phone reimbursement, after 90 days
Monthly in-office lunch to celebrate birthdays, anniversaries, and other events
PTO, paid holidays, after 90 days
Paid sick time
2 work from home days a month, after 90 days
Health, Dental, and Vision insurance, after 90 days
Hours typically from 8:30am-5pm
Mileage reimbursement
