Job Summary:
The Patient Liaison and Shuttle Driver plays a key role within our health education and outreach team, serving as a community leader and advocate.
The Driver works closely with community health workers, health educators, and scheduling dispatchers to ensure that patients, especially seniors and individuals with disabilities, are safely transported to their healthcare appointments.
This position requires professionalism, effective communication, and a strong commitment to serving the underserved Long Beach community.
Essential Job Functions:
* Provide individualized assistance to patients, including senior and/or disabled passengers, utilizing wheelchair lifts and other accessibility features to ensure a safe and comfortable ride.
* Safely load, unload, secure, and transport patients in a mobile shuttle van to their healthcare destinations.
* Adapt to changes in ride schedules and traffic conditions while ensuring patient safety.
* Demonstrate sound judgment and composure when handling emergencies or unexpected changes during transport.
* Follow written instructions, policies, and procedures to maintain compliance.
* Maintain timely communication with the scheduling dispatcher and office as needed.
* Arrive at work on time with a professional demeanor and appearance, adhering to company policies.
* Accurately complete and submit required paperwork in a timely manner.
* Physically assist patients in and out of the vehicle, including handling wheelchairs on paved and unpaved surfaces.
* Manually extend and stow a ramp weighing up to 40 pounds.
* Safely secure mobility equipment and close access doors and panels.
* Maneuver up to 150 pounds as needed to assist patients.
* Perform daily vehicle inspections and clean/sanitize the vehicle after each trip.
* Monitor gas levels and fuel the vehicle when necessary.
* Serve as a trusted liaison between clinic staff, patients, and the community, promoting health access and linkages to services.
* Perform other duties as assigned.
Qualifications:
* Excellent communication and interpersonal skills, with a passion for community health.
* Ability to adapt to changing schedules and handle emergencies calmly and efficiently.
* Physically capable of assisting patients with mobility needs.
* Valid driver's license with a clean driving record.
* Ability to maneuver up to 150 pounds safely.
