Your day as a PCHH RN Case Manager-Sedalia As a new PCHH RN Case Manager at Katy Trail Community Health in Sedalia, you can expect to provide individualized patient care plans, coordinate services, and collaborate with a multidisciplinary team to ensure the best outcomes. While experience in a community health center is preferred, your ability to adapt and learn quickly is essential.
