The Pediatric Care Coordinator and (PCC) promotes improved access and are management for clinic pediatric patients by acting as the liaison between the client and the community, and will help dissolve the silos between the Medical and Behavioral Health (BH) departments within the health centers. This position is an essential member of the primary care team, facilitating streamlined integration of medical, behavioral, and social care for pediatric patients, and coordinating services and follow up with patients and providers. The PCC will also assess patient/family needs, connecting them to preventative care resources/programs, advocating for patients, and otherwise help patients navigate getting the holistic all-around care they need to live healthy and balanced lives from AHS and other agencies.

MINIMUM QUALIFICATIONSHigh school diploma/AA and at least two years of progressively responsible work experience in a community-based setting PREFERRED QUALIFICATIONSBA preferredBilingual in an Asian language is highly preferred.
