Responsibilities: • Refer patients and family members to community resources or to specialists as necessary • Use patient medical information, interviews, observation, and testing as well as collaboration with other staff members to determine a patient's mental health status • Aid with crisis intervention for patients when they are in a state of distress • Monitor each patient situation on a frequent basis and offer the appropriate follow-up care • Work with patients to develop individualized treatment plans with goals and objectives • Assessing patient needs • Medical social workers assess patients' emotional, physical, and psychological needs to understand what they need and how the social worker can support them.
This may include interviewing patients and families to determine their needs • Discharge planning • Medical social workers ensure that patients have a supportive environment to return to and that they have the services they need to function well at home • Developing health care plans • Medical social workers may develop treatment and discharge plans for clients • Educating patients and families • Medical social workers may educate patients and their families • Provide social work services to patients and their families in a medical setting • Conduct assessments to determine the psycho-social needs of patients • Develop and implement care plans based on patient needs and goals • Collaborate with healthcare professionals to coordinate and facilitate patient care • Assist patients and families in accessing community resources and support services • Provide counseling and emotional support to patients and families • Advocate for patients' rights and ensure they receive appropriate care • Document patient interactions and maintain accurate records • Supervise and document limited license medical social workers during counseling sessions • Review and meet with LMSW to discuss their progress Qualifications: • Possess excellent analytical and problem-solving skills, and great communication and interpersonal skills • 1-3 years in a clinical setting working with mental health patients is required • Current and unrestricted state license as an LMHC (licensed mental health counselor) or equivalent • Master's degree in Social Work and active MSW license in [MI] • Minimum of [2] years of social work experience, ideally in-home healthcare or Hospice • CPR certification Compensation: $50 - $65 hourly
