The Perinatal Care Social Worker is a direct care interventionist addressing maternal mental health needs for the perinatal population (pregnant/postpartum). Responsible for providing maternal mental health assessments, brief individual therapy, group intervention, and providing information on community resources to patients within an integrated health care setting. Primary Duties: Provides social work services to identified patients and families including individual and group counseling regarding the impact of illness and hospitalization. Collaborates with other health care professionals. Facilitates communication regarding relevant psycho-social issues of patients and families to health care teams through recording in medical charts, consultation, and participating in meetings. Assess patients to evaluate medical, maternal mental health, psychosocial, financial, and environmental barriers to a healthy postpartum partum period from findings in routine questionnaires facilitated by triage team. Formulates an accurate assessment and develops an intervention plan consistent with the data received during the evaluation. Participates in orientation, training, and educational programs. Facilitates weekly psychoeducational/psychotherapy groups. Provides crisis management when needed and coordinates follow-up care with appropriate parties (consulting with supervisor, community referrals, and/or inpatient units). Qualifications: Master's degree in social work (MSW) from a school of social work approved by the Council on Social Work Education. Successful completion of Social Work practicum as part of professional training. Licensed Graduate Social Worker in the District of Columbia.
