PGY2 Purpose Statement: PGY2 pharmacy residency programs build on Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.
Cardiology: ASHP Accredited: 2007 Duration/Type: 12 months Number of positions: 1 Rotations include, but are not limited to, medical cardiology, cardiothoracic surgery, emergency medicine, infectious diseases, neurology/neurosurgery, pulmonary/critical care medicine, and transplantation. Elective extramural rotations are also offered. Solid Organ Transplant: ASHP Accredited: 2011 Duration/Type: 12 months Number of positions: 2 (1 has been early committed - seeking 1 more through MATCH) Baylor St. Luke's Medical Center performs over 300 adult solid organ transplants for the heart, lung, kidney, and liver transplant candidates annually. There are also coinciding outpatient clinics in each of these areas. We offer a series of rotations designed to provide comprehensive core-learning of diseases and drug therapy issues pertinent to the transplant patient. Rotations will include, but are not limited to, heart transplant, lung transplant, renal transplant, and liver transplant. Elective extramural rotations are also offered. All PGY2 residents have the following experiences and opportunities: Longitudinal residency objectives will be achieved via protocol development, administrative and management projects, completion of research projects, participation in on-call activities and formal presentations. Committee involvement includes national, state and local participation. Residents will be required to staff every third weekend with one major and one minor holiday and a clinical practice month rotation. Residents will have the opportunity to engage in teaching and preceptorship of students during selected rotations. A Certificate in Academia is offered through the University of Houston College of Pharmacy. About the site: Baylor St. Luke's Medical Center (BSLMC), home of the Texas Heart Institute (THI), is located within the Texas Medical Center (TMC) in Houston. THI is consistently ranked as one of the nation's best in Cardiovascular Services & Heart Surgery. Non-profit, quaternary care, adult academic medical center with two campuses licensed for 1040 beds; 44 operating rooms; 164 critical care beds. Department of Pharmacy consists of nearly 150 staff members, of which approximately 80 are pharmacists (1 Director, 2 Managers, 4 Supervisors, 14 Clinical Pharmacist II, 40 Clinical Pharmacists, 7 PGY1 Residents, 6 PGY2 Residents, 2 Fellows) and 60 are technicians with 2 technician coordinators. Adult teaching hospital for Baylor College of Medicine (#1 Medical School in Texas). Active medical staff: Approximately 1700 Physicians. Annual adult admissions: Nearly 25,000 Patients. The applicant must be a highly motivated individual who desires to obtain advanced education and training leading to an enhanced level of professional practice.
The applicant must be enrolled in (or be a graduate of) an ACPE-accredited advanced pharmacy degree program or have equivalent experience. Applicants must register with the NRMP and must adhere to the rules of the Match. Registration with the NRMP will be waived only if a residency position remains unfilled after Phase I and Phase II Match. The applicant should have prior hospital pharmacy experience. Texas pharmacist licensure must be in good standing by the 6th week of residency. Staff every third weekend and clinical practice rotation (4 weeks). Staff at least one major and one minor holiday. Start Residency around July 1st. One major and one minor project. Manuscripts on projects and medication use evaluation. Code Blue participation. On-call responsibility. Case Presentations and Seminars. The application deadlines are during the first week of January. The applicant must submit to PhORCAS the following information: Completed PhORCAS Application. Applications will only be accepted through the Pharmacy Online Residency Centralized Application Service, or PhORCAS. This service may be accessed by visiting www.ashp.org/phorcas. Applicants are responsible for completing all required materials and verifying availability of references by the posted deadline. Official school of pharmacy and/or graduate school transcripts. Curriculum vitae which should include all rotational experiences (completed and pending ones). Three letters of professional references. The reference writers must complete the online information via PhORCAS. The reference writers will need to address the applicant's potential for success in the residency program. References should be from either faculty members in the applicant's professional degree program or from practicing pharmacists with whom the applicant has had work or patient care experience. Letter of intent. A professional photo (e.g. passport photo) of the candidate must be submitted after the applicant has received and accepted the offer to interview. All rules and regulations of the ASHP residency matching program will be strictly followed. Upon Hire Education and Licensure Required: PharmD degree from an accredited school of pharmacy and eligible for licensure in the state of Texas or Registered Pharmacist with the Texas State Board of Pharmacy in good standing-(RPH). Completion of a PGY1 Pharmacy Residency. Aseptic Technique – Complete an ACPE approved 20 hour sterile products course.
