Pharmacy Specialist - Thoracic Oncology - Full Time

GENERAL SUMMARY:

Shift: Monday through Friday, 8:00 AM to 4:30 PM, within the thoracic oncology clinic located in Detroit.
The Clinical Pharmacist Specialist in thoracic oncology will serve as the lead pharmacist and expert in thoracic oncology therapies.
They will work with a multidisciplinary team to provide direct patient care.
They assume leadership responsibility for all pharmacy-related activities in thoracic oncology.
In addition to direct patient care activities, this position is responsible for ensuring appropriate medication therapy and clinical outcomes.
They monitor drug therapy regimens and perform daily in-depth patient profile review to assess, monitor and recommend appropriate drug therapy as applicable based on evidence based medicine, patient characteristics, and financial stewardship.
They will consistently evaluate, intervene, and document adverse drug reactions, medication variances and clinical interventions beyond institutional protocols.
They prevent and/or resolve drug-related problems to ensure safety, efficacy, and cost-efficiency by consulting with the ordering prescriber and appropriately documents.
They answer drug information questions with in-depth knowledge of research designs, methodologies, and biostatistics.
They will precept pharmacy trainees (students and residents) along with research projects, continuing education lectures, etc.
every year.
They will oversee the work of trainees and technicians to ensure performance standards are maintained and work is in accordance with state, federal and organizational regulations.
They will be responsible for developing clinical guidelines, policies, and protocols in thoracic oncology.
They will serve on related committees for the system and oncology organizations at the state and national level.
They will review new clinical trial protocols for thoracic oncology, support clinical staff in infusion and oncology, transitions of care coordination as needed, regulatory compliance monitoring, presentations, quality improvement projects, precepting residents and students, utilizing data to track, improve, and share patient outcomes, and present and publish their work.
PRINCIPAL DUTIES AND RESPONSIBILITIES:

In addition to fulfilling all other duties and responsibilities of a pharmacist, the Thoracic Oncology Pharmacy Specialist will:

Serve as the lead pharmacist for all health care professionals and patients with thoracic oncology needs.
Assume overall leadership responsibility in directing all pharmacy-related activities in thoracic oncology by planning, developing, and implementing new and/or improved systems or services.
Collaborate with the medical and surgical oncology teams to provide optimal patient care.
Stay abreast all thoracic oncology therapies, guidelines, and literature along with their potential relevance for the organization.
Oversee and manage the onboarding of all thoracic oncology therapies recommended for formulary inclusion with front-line stakeholders, including but not limited to policy development, committee approvals, education.
Develops all thoracic oncology care pathways or algorithms for pharmaceutical use within the electronic health record and/or drug use policy and ensures that these are kept up to date and are communicated to the respective providers and stakeholders.
Patient Care Services: Coordinate & connect with stakeholders; Track patient progress through phases and encourage timely movement along the patient journey; Provide single pharmacy source of communication; drug/clinical information.
Monitors all requests for thoracic oncology therapies and coordinates their use at Henry Ford Health, which includes working with:

Patient Billing

Finance to ensure business case approval when applicable

Pharmacy Supply Chain to arrange for ordering

Infusion Center to help the clinic coordinate access of medication and supplies, education for preparation, administration, and monitoring, and scheduling.
Provider(s) in the clinic to assist with patient coordination activities

Any other coordination that is necessary (i.e.
policies, committee approvals)

Reviews requests for thoracic oncology therapies to determine if the request is consistent with payor plan guidelines.
Completes literature searches and reviews with clinical team to determine medical necessity and coverage.
Assists medical staff in making an appeal for reconsideration when a request is denied.
Applies evidence-based medicine and financial stewardship when assessing therapies for patients

Focuses on lowering overall health care costs and financial stewardship for patients and the organization.
Applies clinical trials when determining most appropriate therapies for patients.
Outcomes: Track and monitor outcomes, such as efficacy and safety, as well as all medication related requirements for regulatory and contractual compliance.
Participate and support pharmaceutical research through precepting pharmacy residents and students, as well as through publications or presentations to advance knowledge and practice in this area.

EDUCATION/EXPERIENCE REQUIRED:

Must hold a degree from an accredited college of pharmacy and must complete a residency training program or have equivalent experience in hospital pharmacy practice.
Must have completed an ASHP accredited PGY1 and PGY2 Pharmacy Residency or equivalent experience.
Minimum of 3-5 years of pharmacist experience or the equivalent training and experience at another institution, preferred.
Must have demonstrated the ability to meet and exceed practice standards set for a Level II pharmacist.

Must be able to serve as an institutional authority in a given specialty practice area.
Must have demonstrable leadership abilities and exhibit teamwork when working with others.
Must have excellent verbal and written communication skills which enable him/her to contribute effectively in the dissemination of information to all levels of health care personnel in the institution.
Should have a proven research/publication record.
Must be qualified to work with pharmacy students at the undergraduate and graduate levels.
Must have a previous work history of reliability, dependability, and willingness to assume additional responsibilities.
CERTIFICATIONS/LICENSURES REQUIRED:

Must be licensed or eligible for licensure in the State of Michigan.
If eligible, licensure must be obtained within 30 days of hire; failure to obtain licensure will result in termination of the Pharmacy Specialist position.
