*$20,000 sign on bonus!**Davis Place, *a beautiful newly renovated skilled nursing facility in *Danielson CT,* is seeking a *Physical Therapist* to join their therapy team.*Qualifications:** Graduate of an accredited university with a B.S., MA, or M.S. in Physical Therapy that is recognized by the APTA.* Holds current license and/or registration in the state of CT (as applicable)*Job Responsibilities include but are not limited to:** Provide a comprehensive treatment plan including long- and short-term goals, frequency, duration and treatment modalities.* Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated while adhering to precautions.* Document patient's progress on weekly progress note accurately and timely.* Document monthly updated treatment plan on standardized form in timely manner.* Document any caregiver or discharge in-services on an in-service form as indicated.* Document care plans and information needed for the MDS* Attend facility and rehab meetings as needed.*Job Type:* Full-timeJob Type: Full-timeBenefits:* 401(k)* Dental insurance* Health insurance* Paid time off* Retirement plan* Vision insuranceSchedule:* 8 hour shift* Monday to Friday* Weekends as neededLicense/Certification:* Physical Therapy License (Required)* CPR Certification (Required)Work Location: In person
