Job Title: Physical TherapistDepartment: Outpatient Physical MedicineLocation: Putnam, CTHours: 32/week - part timeShift: 1stPhysical Therapist Job Summary:Under the general direction of the Physical Medicine Director organizes and administers prescribed physical therapy programs in an effort to reduce and/or prevent disability to help patients achieve their maximum functional potential.Physical Therapist Home Care Essential Responsibilities:Evaluates and treats patients as prescribed by referring physicianRecords treatment, response, progress and discharge information in patient's chart or enters information into computerEducates patient, family and significant others regarding condition, treatment and home programRoutinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of planOrients, instructs, and directs assistants, aides and studentsMaintains professional growth by attendance of department meetings, inservice and educational conferences. Includes participation in hospital orientation and departmental and community inservice education and programs as directed.Assists with the planning and implementation of new programsPhysical Therapist Education Qualifications:BS, MS or DPT degree from an accredited program in Physical Therapy required. Licensure by the state of Connecticut required.
