Full Time Physical Therapist Case Manager, Level 2 Anticipated Territory: Sacramento County: Elk Grove and Surrounding cities* In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment
Your goals as a Physical Therapist Case Manager, Level 2 are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs.
You will also supervise 2 Physical Therapy Assistants (PTAs) ensuring they are meeting performance standards.

\n$135,000 - $150,000 a year
Wage range Physical Therapist, Level 1: 130,000.00- 150,000.00 plus 20K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.  21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match).
