We are seeking a Full-Time Physical Therapist (PT) to join our dynamic team. This is an excellent opportunity for a motivated and skilled PT to enhance residents' mobility, strength, and quality of life while working in a positive and professional setting.
Position Summary:The Physical Therapist (PT) is responsible for evaluating residents' physical needs, developing individualized treatment plans, and providing therapeutic interventions to restore function, improve mobility, and reduce pain. This role involves collaborating with an interdisciplinary team to achieve optimal outcomes for our residents.
Key Responsibilities:
Conduct comprehensive evaluations of residents' physical abilities and limitations.
Develop and implement personalized treatment plans to enhance residents' mobility, strength, and independence.
Provide therapeutic interventions, including exercises, manual therapy, and use of assistive devices.
Monitor residents' progress and modify treatment plans as necessary to achieve goals.
Collaborate with the interdisciplinary team, including physicians, occupational therapists, and nursing staff, to ensure coordinated care.
Educate residents and families on therapy goals, home exercises, and strategies to maintain progress.
Document evaluations, treatment sessions, progress notes, and discharge plans in accordance with facility and regulatory requirements.
Participate in team meetings and contribute to quality improvement initiatives within the rehabilitation department.
Qualifications:
Doctorate or Bachelor's/Master's in Physical Therapy from an accredited program.
Current and valid Physical Therapist license in the state of Colorado.
Experience in a skilled nursing or rehabilitation setting preferred.
Strong interpersonal and communication skills with a collaborative approach to care.
Proficiency in documenting and utilizing electronic medical records (EMRs).
