What does a Pinnacle PT do?
As a Home Health Nurse for Pinnacle Home Care, you will travel to a patient's home to administer services and provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. Some tasks include:

Complete an initial assessment of patients and determine home care needs including Outcome and Assessment Information Set (OASIS) assessments.
Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
Teaches, supervises, and counsels the family and client in the physical therapy program.
Supervises Physical Therapist Assistants at least every 30 days.
Communicates with the physician whenever changes occur regarding the client's status and obtains additional orders, if necessary.
Communicates with other team members (including Home Health Aides) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
Writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.

 
Qualifications:

Graduate of an accredited Physical Therapy program or equivalent education.
Licensed to practice as a PT in the State of Florida.
Current CPR Certification.
Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
At least one year of experience as a Physical Therapist.
Home healthcare experience is preferred but not essential.
Electronic medical record experience is preferred but not essential.
Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations. Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
