Location/Department:Rehab Center - Physical TherapyPRNJob Summary:Provides physical therapy to inpatients and outpatients.Is responsible for hands-on care, as well as associated documentation.Operational tasks of the department are required as well as participation in development.Supervisory Responsibilities:NoneDuties/Responsibilities:Selects appropriate evaluation tools.Administers evaluations correctly & safely.Uses correct documentation tools for patient types.Demonstrates competency in assessment of inpatients & outpatients.Develops appropriate plan of care for patients.Develops appropriate objective & measurable goals.Develops effective & therepeutic based treatment plans.Initiates & progresses treatment plans according to patient progress level.Performs treatment in a safe manner for the patient & for self.Demonstrates good time management in planning & application of care plans.Applies & modifies treatment based on continual assessment of patient performance, skills, patterns, & relevant factors.Maintains effective communication with physicians, case managers, patients, caregivers, family, & other health care staff.Communicates effectively & appropriately with team members.Performs timely documentation using appropriate terminology & format.Effective use of EHR & other hospital systemsEnters appropriate charges for treatment administered.Active participation in patient care meetings as appropriateRepresents the PT department in interdepartmental meetings as appropriate to the work setting.Performs patient & family education routinely as part of therapeutic intervention.Incorporates written instructions & home programs for patient & family reference as needed.Provides in-services to other MMH team members or to the community.Participates in departmental meetings & in-services.Participates in outside workshops & continuing education opportunities.Actively involved in departmental program improvements, new/specialty program developmentsActively involved in new team member orientation.Provides clinical supervision of PTA, aides, students, or volunteers.Participates in peer/competency review activitiesKnows federal & state regulations pertaining to delivery of PT services in various settings.Knows Medicare, Medicaid, private insurance, and workman's comp regulations as they apply to the delivery of PT services.Follows MMH & PT policies & procedures in the performance of work activities.Required Skills/Abilities: Strong customer service skillsEffectively communicates both verbal and writtenGeneral physical disability treatment skills for physical therapy patients.Education and Experience:Bachelor's degree in Physical TherapyLicensed as a Physical Therapist in IndianaNo experience requiredPhysical Requirements: Frequent walking, standing, reaching, talking, & use of handsMay need to lift up to 25 lbs; occasionally up to 50 lbs
