Physical Therapist

International Quality Homecare is seeking a full-time, part-time, or per diem Physical Therapist (PT) to work out of our Ellsworth, WI location.




*Basic Qualifications:*




The Physical Therapist must be licensed in the state of practice and be free from any disciplinary action.
The PT is expected to have a valid driver's license.




*Other Qualifications:*




The position requires a clear understanding of therapy in the Homecare setting.
The ability to assess the needs of the clients and work with individuals within and outside the company to meet the needs of the client is required.
The position requires a demonstration of strong analytical, problem-solving, communication, and leadership skills.
Computer proficiency and the ability to learn different computer programs are highly required.




*Physical Requirements*




· Lift, push, and/or pull a minimum of 50 lbs




· Extended periods of walking, standing, and driving required




*Responsibilities:*




Evaluates and implement a physical therapy program on the referral of the client's physician.
Provides written evaluation and plan of care, Writes progress notes and discharge summaries according to departmental policy.




Coordinates physical therapy with nursing staff and other rehabilitative services.




Attends require educational in-services to meet license renewal requirements.




Supervision of PTA and delegation of the treatment plan.




Will follow all appropriate agency infection control policies and procedures.




Observes, record, and report to the physician the client's reaction to treatment and changes in the client's condition.

Willingness to keep up with current physical therapy practices.




Ability to exercise independent judgment.
