Vision: Committed to sharing resources, expertise, and vision for a healthier community. Core Health Partners (CHP) is seeking a full-time Physical Therapist to fulfill the following job duties: 1. Evaluate patient needs and create treatment plans 2. Provide and oversee physical/aquatic therapy services 3. Direct and instruct patients with chronic disease conditions, physical disabilities, or injuries 4. Supervise a team of: - Physical Therapist Assistants (PTAs) - Aides - Student interns Patient Population - Work with patients across the lifespan (adult and pediatric), however we are able to customize to preference and skill set. This distinctive methodology demands creativity, adaptability, and collaboration to effectively address the needs of our patient-centered care pathway.JOB DESCRIPTION- Core Health Partners Professional Relationships – Must be able to develop and maintain a strong relationships with a network of referring physicians. I. Job Responsibilities: - Deliver high-quality patient care services in accordance with professional guidelines and code of ethics - Conduct thorough examinations to develop personalized plans of care that: - Promote movement and function - Reduce pain and discomfort - Prevent disability and disease progression - Collaborate with patients, family members, and healthcare providers to achieve optimal outcomes - Assess patient needs and create comprehensive treatment plans - Provide and oversee physical and aquatic therapy services for various conditions, including: - Musculoskeletal conditions (e.g., back pain, joint injuries) - Developmental delays (e.g., pediatrics, neonatal care) - Chronic diseases (e.g., diabetes, obesity, Parkinson's, arthritis, Alzheimer's, vascular disease) - Direct and instruct patients to achieve maximum functionality and independence, empowering them to maintain wellness goals and a clear path to long-term success after discharge. I. Data Management and Reporting - Collect and report data to support patient care and outcomes - Develop instructions and reports to assist caregivers and community non-profits in continuing patient care beyond medical billing II. Collaborative Care - Contribute to maintaining patient wellness through collaborative efforts III. Compliance - Comply with Florida Board of Professional Licensure requirements and supervisory rules and regulations IV. Communication - Demonstrate effective written, verbal, and non-verbal communication with patients, staff and collaborating agencies operating in this care model - Protect private health information - Communicate timely with referring physicians regarding patient treatment plans, progress notes, and discharge V. Documentation - Complete all documentation and charting in a timely manner within the work day VI. Equipment Management - Operate equipment related to physical therapy procedures and treatments - Follow equipment maintenance protocols and report potential problems to supervisor. Must also be able to operate without the equipment of a sick based model of care and innovate to provide quality treatments in a well-based setting. VI. Patient Education - Empower patients to adopt healthy lifestyles by creating personalized at-home fitness and wellness program plans - Develop and maintain a library of generic program plans on Core Health Partners' HIPAA compliant Employee Portal, enabling clinical staff to: - Review and learn from successful programs - Adapt plans to suit individual patient needs - Promote continuity of care and best practices. VIII. Staff Support - Serve as a staff point of contact for general assistance and emergency situations, hold team meetings with minutes and use technology to keep staff informed. IX. Other Duties as Assigned - As a collaborative team, all staff members are expected to embody the company's mission and integrate our innovative care model into the community setting. Additionally, staff may be assigned other duties that support the company's mission and contribute to its success. These duties may include, but are not limited to: - Participating in community outreach and engagement initiatives - Collaborating with community partners and stakeholders - Identifying opportunities for growth and improvement - Contributing to special projects and initiatives - Supporting cross-functional teams and initiativesMINIMUM ENTRY REQUIREMENTS: - Active, unrestricted license to practice as a Physical Therapist in the state of Florida - American Red Cross certifications in: - CPR-/AED/First Aid - Blood Borne Pathogens Training - Proficient computer skills, including: - Microsoft Office (Word, PowerPoint, Excel) – Ability to be proficient in navigating our Electronic Health Record (EHR) documentation system of software. Documentation: Accurate and timely documentation of patient records and treatment plans. 8. Critical Thinking: Ability to analyze data, make sound judgments, and adjust treatment plans accordingly. 9. Problem-Solving: Skilled in resolving complex patient problems and adapting to changing situations. 10. Empathy and Compassion: Demonstrating care, empathy, and understanding when working with patients. Desirable Skills: 1. Specialized Knowledge: Expertise in specific areas, such as pediatrics, geriatrics, sports physical therapy, or wound care. 2.
