Evaluate patient needs and create treatment plans 2. Provide and oversee physical/aquatic therapy services 3. Data Management and Reporting - Collect and report data to support patient care and outcomes - Develop instructions and reports to assist caregivers and community non-profits in continuing patient care beyond medical billing II. Must also be able to operate without the equipment of a sick based model of care and innovate to provide quality treatments in a well-based setting. VI. Patient Education - Empower patients to adopt healthy lifestyles by creating personalized at-home fitness and wellness program plans - Develop and maintain a library of generic program plans on Core Health Partners' HIPAA compliant Employee Portal, enabling clinical staff to: - Review and learn from successful programs - Adapt plans to suit individual patient needs - Promote continuity of care and best practices. VIII. Staff Support - Serve as a staff point of contact for general assistance and emergency situations, hold team meetings with minutes and use technology to keep staff informed. IX.
