Plans implements and evaluates client care plans to restore or maintain patient well-being. Plans organize and conducts physical therapy treatment based on the medical referral and evaluation.*Job Responsibilities:*· Promote /exemplify the Company mission, vision and values at all times.· Perform clinical assessments; identifies patient needs and appropriateness of client.· The document, prepare/revise and completes a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.· Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).· Plan and provide appropriate treatment for neuro-muscular, neurological and orthopedic disorders based on medical referral and therapy assessment.· Assist/instruct client/family in transfer techniques appropriate to client abilities. Instruct on use of equipment.· Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case conferences/Team Meetings.· Monitors assigned cases to ensure compliance with requirements of third party payers.· Completes appropriate documentation in a timely manner to assure compliance with company policy.· Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.· Meet mandatory continuing education requirements of the Agency/licensing board.*Experience:** Two (2) years' experience as a PT in a clinical care setting.* Preferred, Home health experience.*Home Health PT Requirements:*· Current State professional license as a PT in any/all applicable states· Current CPR· A valid driver's license and reliable transportation, insurance required.· Therapist skills as defined as generally accepted standards of practice.· Excellent observational and problem-solving, decision-making and report writing skills.· Excellent interpersonal skills and ability to communicate effectively.*Education:*· Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.#INDZYJob Type: Full-timePay: From $85,000.00 per yearBenefits:* 401(k)* 401(k) matching* Dental insurance* Employee assistance program* Flexible schedule* Health insurance* Life insurance* Paid time off* Retirement plan* Vision insuranceMedical Specialty:* Home HealthSchedule:* 8 hour shiftExperience:* Physical Therapists: 1 year (Required)License/Certification:* Physical Therapy License (Required)Work Location: In person
