Physical Therapist

Job Summary: Middletown Park Rehabilitation & Healthcare is seeking a skilled Physical Therapist to join our team.
As a Physical Therapist, you will be responsible for planning, organizing, and implementing physical therapy services to help residents achieve their highest level of physical, mental, and social well-being.
Duties and Responsibilities:
Assist in planning, developing, and implementing physical therapy services in accordance with federal, state, and local standards, guidelines, and regulations.
Develop and implement programs, goals, objectives, policies, and procedures to help residents maintain their highest level of physical, mental, and social well-being.
Interpret and instruct physical therapy policies and procedures to personnel, residents, and family members as necessary.
Participate in discharge planning, development, and implementation of resident care plans and resident assessments.
Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
Maintain treatment records, resident files, and progress notes as required.
Requirements:
Current license and registration to practice as a Physical Therapist in the State of NY.
Current malpractice insurance and license in good standing.

Ability to work well under pressure and in a team environment.
