Northern Light Sebasticook Valley Hospital-Pittsfield, ME Department: Physical Therapy Work Type: PRN FTE: 0.0001: As Needed Work Schedule: 8:00 AM to 4:30 PM Summary:

To provide goal-oriented patient directed physical therapy assessments and therapeutic care, commensurate with hospital policy and all licensing and regulating authorities.
To provide comprehensive evaluation, treatment and education to physical therapy patients in order to assist the patient in achieving their highest level of function.
Participates in rehab department PI, department meetings and hospital wide activities to promote staff development and patient safety.
Assist with the development of department strategic goals that assist with the achievement of SVH's mission and vision.


Responsibilities:

PATIENT ASSESSMENT •Demonstrates proficiency and competence during patient assessment activities relative to physical therapy scope of services, professional practice standards and all applicable licensing and regulatory agency.
•Adheres to clinical documentation requirements for physical therapy in a manner that ensures continuity of care and accurately reflects patient's function, progress toward goals and justifies intervention worthy of reimbursement.
•Demonstrates sound judgment and competence in selection of appropriate diagnostic/clinical evaluation procedures in accordance with patient's needs, age and development level.
•Completes thorough evaluations of the patient's functional status in accordance with department standards that translate to appropriate clinical treatment strategies.
•Accommodates varying learning styles and communicates appropriately with patients of varied background and/or needs.
•Incorporates clinical observation in standardized evaluation processes and procedures.
•Discharge needs are assessed at the time of the evaluation and patients are included in the goal setting.


PATIENT CARE •Develops an individualized physical therapy treatment plan in consideration of the patient's strengths, problems, clinical prognosis, rehab potential, discharge needs and patient's/caregiver's goals.
•Establishes realistic measurable short and long term goals based on evaluation results and patient/caregiver goals or concerns.
•Completes progress notes in a timely fashion and in accordance with department expectations and standards.
•Selects, prescribes, instructs and continually reassesses treatment effectiveness when performing treatment procedures and therapeutic modalities.
•Updates and revises treatment plans and goals according to patient's progress.
•Demonstrates competence in timely completion of all documentation requirements.
•Adheres to clinical documentation requirements in a manner that ensures continuity of care and accurately reflects patient's function, progress toward goals and justifies intervention worthy of reimbursement.
•Completes discharge documentation in a timely manner.


PATIENT AND FAMILY EDUCATION •Collaborates with the patient and family throughout treatment to enhance the outcome and determine the effectiveness of the prescribed treatment.
•Consistently educates the patient and family throughout the treatment process to enhance their understanding and ability to participate in the prescribed treatment.
•Maintains open communication regarding patient's status (verbal and written) with the referral source as necessary.
ENVIROMENT OF CARE •Able to respond appropriately to all hospital codes.
•Demonstrates correct technique and proper use of equipment.
•Assists with the maintenance of a safe and clean environment.
•Is competent in completing work requisition forms and/or reports repair needs to the department manager per policy.
PERFORMANCE IMPROVEMENT •Participates as an active member of the department PI program and its initiatives.
•Recognizes, responds to, and reports conditions that require an improvement in performance and/or process.
STAFF AND SELF DEVELOPMENT •Attends and actively participates in monthly staff meetings.
•Actively participates in the department's student program and serves as a professional mentor.
•Constantly seeks and avails self to learning opportunities that promote clinical competence and professional growth.
•As requested, participates in hospital wide committees and activities and positively represents the department.
•Ensures clinical compliance with mandatory in-services, licensing renewals, BLS for Healthcare Providers renewals and compliance with policies and procedures.

•Demonstrates initiative and independence when performing non-assisted duties.

PHYSICAL THERAPIST RESPONSIBILITIES •Demonstrates an understanding of the department's strategic plan and conducts self in a fiscally responsible manner.
•Is competent in requesting and ordering necessary supplies, support materials and patient equipment needs.
•Actively participates in promoting department growth and meeting administrative requirements of the hospital and department.
•Completes daily patient charges and billing procedures that accurately reflect services provided.
•Continually strives to keep abreast of insurance requirements.
•Maintains open lines of communication with manager and department staff to ensure achievement of common goals.
•Recognizes and successfully intervenes and/or seeks appropriate
