Part-time Onsite Position $10,000 Bonus Opportunity Overview: In this vital role, you will be instrumental in delivering physical therapy services directly in patients' residences, enhancing their functional capabilities and overall quality of life. Your expertise will help shape individualized treatment plans, facilitate patient and caregiver education, and ensure continuous improvement through effective supervision and meticulous documentation.
Qualifications: Required: A Master's degree or higher in Physical Therapy. Valid Physical Therapist license in Illinois. A minimum of one year experience within a healthcare environment. A valid regular Driver's License. Preferred: Kindly include your PT license number with your application. Key Responsibilities: Develop and implement comprehensive treatment programs following thorough assessments of patient conditions. Instruct both patients and caregivers on the correct use of therapeutic devices and prescribed therapy programs. Accurately document treatment reactions and any changes in the patient's condition. Actively participate in professional development activities within the staff and agency. Oversee and mentor Physical Therapy Assistants and, when required, Certified Nursing Assistants. Schedule and Shift Details: This full-time role requires local travel within a one-hour radius of Springfield, IL, to deliver services in residential settings. The schedule includes working Monday to Friday from 8:00 AM to 5:00 PM, with one weekend per month and two holidays per year.
Candidate Details: We are looking for candidates who are dedicated to fostering therapeutic success in home settings, have a solid grounding in physical therapy practices specific to adult care, and who are adept at managing both individual caseloads and contributing to team objectives.
