Optimal Care, Inc. is seeking a Full Time Physical Therapist committed to providing the highest quality care in our service area.
We offer competitive visit rates and excellent working conditions.
Home care experience is preferred, but not required.
Physical Therapist Responsibilities: Provides skilled physical therapy services in accordance with the physician's plan of care.
Assists the physician in assessing the client's functional level by applying appropriate tests and assist in the development and revision of the plan of care, including OASIS assessments and goal setting as appropriate.
Treats clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.
Identifies barriers to desired outcomes.
Observes, records, and reports the client's reaction to treatment and any change in the client's condition to the physician and/or client care manager and other staff involved.
Instructs and advises client, family, and other health team personnel; instructs in use of prosthetic and orthotic devices and DME as appropriate.
Cooperates and consults with appropriate staff to provide staff education when requested and arranged by the client care manager.
Evaluates and regularly reevaluates the rehabilitation and equipment needs of the client; initiates, develops, implements, and makes necessary revisions of the client's plan of care.
Assesses the client's continual care needs.
Ensures that the plan of care addresses all problems identified in the assessment or demonstrates rationale for not doing so.
Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed or as changes in the client condition require.
Assists with the coordination of the total plan of care and maintains continuity of client care by liaising with other health professionals assigned to the same clients.
Attends client care conferences.
Initiates client care conferences for complex and/or multidisciplinary clients when required or helpful to ensure good coordination of care.
Supervises and teaches the home health aide.
Assesses the client's need for unskilled care every two (2) weeks and revises the plan of care as appropriate.
Communicates plan of care changes to the aide and scheduler as often as necessary.
Supervises and coordinates with Physical Therapy Assistants (PTAs) as necessary.
Assesses the client's need for skilled care every thirty (30) days and revises the plan of care as appropriate.
Communicates plan of care changes to the PTA and scheduler as often as necessary.
Develops, prepares, and maintains individualized client care progress records with accuracy, timeliness, and according to policies.
Submits accurate and required documentation daily.
Start of care forms to be submitted within 24 hours of initial visit.
Participates in discharge planning as indicated.
Participates in the agency's quality assurance performance improvement program.
Keeps abreast of physical therapy trends and knowledge for service provision, documentation, and care coordination.
Participates in in-service programs.
