You will evaluate the patient's home environment and functional status and participate in the development of the total plan of care.Develop a treatment program and establish goals for patients improved functionTeach patients new skills or re-train patients in once familiar daily activities that have been lost due to illness or injury, under organization policyMaintain and submit appropriate clinical records, notes, and reports to the physician regarding any changes in the patient's conditionLicensed to practice as Physical TherapistMinimum of one (1) year of experience as a Physical TherapistGraduate of an accredited Physical Therapist (PT) therapy program - new grads welcome!Current state licensureExcellent verbal, written and interpersonal communication skillsResults-oriented with strong critical thinking and problem-solving skillsTech-savvy with the ability to learn quicklyAre you ready to join our team?Are you caring, kind, and compassionate?
