Job DescriptionThis position is located at our Woodland, Gibson Road location.POSITION SUMMARY: Under general supervision from the Lead Medical Provider, the Physical Therapist is responsible for managing referrals from both internally generated patient care as well as external patient care providers.
A successful incumbent will work as part of a care team for NVIH patients by collaborating with medical providers and support staff to create an environment to help patients improve through physical therapy techniques.ESSENTIAL JOB FUNCTIONS: The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities.
Incumbent(s) will be required to perform a variety of duties listed, including but not limited to:Provide direct physical therapy services to program patients, including, but not limited to: obtaining/reviewing and documenting appropriate patient history, making and documenting diagnoses/assessment, develop and document an individualized treatment plan, assess and document progress, and communicate appropriately with referring providers.Maintain equipment and work area in clean and safe condition.Initiates physical therapy treatment plans and instructs other personnel and caregivers, as appropriateTreat patients in a Respectful and Compassionate manner to motivate and inspire them to continue with treatment plans for improved health outcomes.Assisting with development of programs and policies relating to physical therapy.Participate and assist in quality assurance and peer review processes as directed.Other duties as assigned.All NVIH employees are expected to: - Provide the highest possible level of service to patients;- Promote teamwork and cooperative effort among employees;- Maintain safe practices; and- Abide by current NVIH's policies and procedures.Experience and SkillsREQUIRED QUALIFICATIONSThe following knowledge, skills, and abilities are required for an employee to successfully perform their duties.Maintain current Physical Therapy licensure in California.Five (5) years' experience required in a Physical Therapist capacity.Knowledge of medical operations as they pertain to managing staff and patient care, scheduling and billing, and compliance regulations.Ability to apply skills and knowledge to provide relevant solutions and feedback to senior leadership and other colleagues for the betterment and advancement of the Agency, patients, and employees.Demonstrates understanding of age-related or specialty issues and developmental needs for each population served by adapting style, treatment and plan to each population.Exhibit empathy and customer service to diffuse and manage conflict within the team and with patients or their representatives.Excellent written and oral communication skills, strong organizational problem-solving and analytical skills.Skill in establishing and maintaining positive and effective working relationships with department directors, and others contacted in the course of the work.Reliable transportation and ability to travel between sites as necessary.Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm as an innovative team player.Indian Preference.
