The Physical Therapist (PT) administers skilled physical therapy to patients, in their home, who require intermittent skilled therapy service. She/he collaborates with the other staff involved in the patient's care to render the best possible care and to achieve the best outcomes. The therapy services are performed in accordance with the physician's orders. The Physical Therapist is supervised by and receives direction from the Assistant Director of Nursing.*Requirements*:* Must possess and maintain a current Physical Therapy license in the State of New Mexico;* Must be able to render care under the accepted standards of practice;* At least two (2) years' experience as a Physical Therapist in any clinical setting, preferred;* Must demonstrate competency in taking basic vital signs: blood pressure; temperature, pulse, blood glucose;* Must participate in other competency training as deemed necessary for your position;* Must be able to provide proof of right to work in the United States;* Working knowledge of navigating computers and using a word program preferred;* Must have a current driver's license, proof of automobile insurance, satisfactory driving record and reliable transportation;* Will be evaluated at least annually by your supervisor;* Must be prepared to drive to remote areas up to 300 miles away from the office;* Must have at least 12 hours of In-Services annually* Must have CPR certification and PPD current.* Must be able to pass a criminal background check;* Must be willing to, at any time during working hours; participate in a random alcohol/drug screening.Job Types: Full-time, Part-timePay: $40.00 - $50.00 per hourExpected hours: 40 per weekBenefits:* 401(k)* 401(k) matching* Dental insurance* Life insurance* Vision insuranceMedical Specialty:* Home HealthSchedule:* 8 hour shift* Monday to FridayLicense/Certification:* CPR Certification (Preferred)* Physical Therapy License (Required)* Driver's License (Required)Work Location: On the road
