We are looking for exceptional Physical Therapist to perform therapy visits for our pediatric patients.

A registered professional who provides coordinated care to patients of all age groups.  Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation. A licensed and certified physical therapist (PT) will provide therapeutic programs to pediatric patients with cognitive, physical, and sensory disabilities or delays in the home. The PT plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Participates in the coordination of care. 
Salary range:
Salary Range: $70,000 to $90,000 annually
 
Responsibilities:• Evaluates patients with physical, psycho-social and/or cognitive impairments to determine if occupational therapy can benefit the patient's quality of life. • Plans, implements, and evaluates patient care plans in conjunction with the physician and care team to restore or maintain patient well-being. • Thoroughly documents care provided and participates in the coordination of care with other care team members.• Provides education and training to patient caregivers and family.• Provides supervision to Certified Occupational Therapist Assistants and Aides, when applicable, in the performance of patient care that is consistent with the patient's needs. 
Qualifications:• Current State License as an Occupational Therapist • Current driver's license and proof of current CPR• One year experience as an Occupational Therapist preferred
