Job Description Summary
Physical Therapist Assistant works under the supervision of a licensed Physical Therapist to provide physical therapy services to patients in their own homes. The PTA assists in implementing treatment plans designed to improve mobility, reduce pain, restore function, and prevent disability. The role involves traveling to patients' homes, delivering therapeutic exercises, and providing patient education to ensure effective and safe care in a home environment.
Essential Job Functions/Responsibilities
1. Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
2. Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity.
3. Teaches, supervises, and counsels the family and client in the physical therapy program.
4. Instructs client in care and use of adaptive devices such as: wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
5. Arranges for out-patient service for procedures when appropriate.
6. Records on a timely basis all treatments, and client's response to therapy intervention Also records quality assurance data.
7. Communicates with PT and other team members (including Home Health Aides), when appropriate. Instructs them in certain phases of physical therapy that they may use while working with the client
8. Under the direction of a Physical Therapist, writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.
9. Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
10. Participates in effective coordination of schedule to ensure all reassessments are performed in a timely manner.
11. Obtains a medical history, particularly as it relates to the present condition.
Under the supervision of the physical therapist, assists in evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
As directed, performs routine strengthening and therapeutic exercises and assists the patient with the use of self-help devices.
Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the PT.
Observes, and records activities and findings in the clinical record and reports to the PT and the physician any adverse reaction to treatment.
Participates with staff, clients, and physicians in discharge activities and assists the Physical Therapist in completing the Physical Therapy Discharge Summary.
Assumes other duties deemed appropriate and necessary by the PT and as designated by Supervisor
Awareness of the Agency's QAPI program. Active participation as required per policies and procedures related to the Agency's QAPI program.
Management and follow up of customer complaints, following Agency's complaint policy
Acute awareness of patient rights and the Agency's responsibility in honoring patient's rights. Awareness of patient rights description in Agency's service agreement and Agency's admission packet.
Awareness of appropriate reasons for a patient discharge and Agency's policy on appropriate reasons for discharge.
Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.
Awareness and ability to design and identify patient centered and individualized goals with realistic measurable timeframes under the supervision of the Physical Therapist.
Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency's policy and procedures. Awareness and participation in the Agency's infection control program.
Appropriate bag technique policy acknowledgement and demonstrated competence when applicable.
Physical therapy equipment and other medical equipment will be utilized. Hand washing materials and PPE.

Position Qualifications
· The ability to drive and the ability to access clients' homes, which may not be routinely wheelchair accessible are required.
· Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
· May be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
· Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants.
· Must be licensed by the State as a Physical Therapist Assistant.
