Bradford Hills Nursing and Rehab Center, located in Troy, PA seeking full-time PTA for in-house rehab team!Ask about PRN Opportunities!Job Description:The Physical Therapy Assistant (PTA) works under the supervision of the Physical Therapist to implement treatment plans and assist residents in their rehabilitation journey.Responsibilities:· Assist the Physical Therapist in carrying out treatment plan and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.· Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.· Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.· Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( InnovaHealth/Net Health/PCC).· Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.· Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.Qualifications:· State licensure as a Physical Therapist Assistant (PTA).· EMR and MS office applications.· CPR/BLS certification.· Knowledge of Section GG data collection and documentation requirements.· Strong clinical and interpersonal skills.· Excellent written and verbal communication skills.· Ability to supervise assistants, technicians/aids and student/interns.· Modality Certification +· Experience in LTPAC/LTC clinical programs are always a plus
