Physician Assistant / Nurse Practitioner

We will work with you to set the ideal schedule that works best for you in one or more patient centers.*Responsibilities* Function autonomously to care for patients with complex, acute, critical and chronic health conditions.* Prescribe medications including the frequency and dosage* Work individually and in interdisciplinary teams of resources and consultants* Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers* Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams* Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further careSkills* Demonstrated problem-solving and workflow management* High level of clinical proficiency and excellent decision-making skills.* Ability to work independently and as an effective member of a healthcare team.* Excellent oral and written communication skills.* Ability to effectively collaborate and build relationships with patients, families and all care providers* Proven organizational skills and ability to prioritize effectively.* Strong computer skills with knowledge and experience with Electronic Medical RecordsQualifications* Board Certified as a FNP* DEA Substance License required* BCLS/ACLS certification required* Prior APN experience preferredJob Type: Part-timePay: $100.00 - $150.00 per hourExpected hours: No more than 10 per weekBenefits:* Flexible scheduleEducation:* Master's (Preferred)Work Location: In person
