Position Highlights: 100% Outpatient Join current team of 2 Physicians & 7 APP's Office hours are Monday–Friday 7:00am-5:00pm (4 Day Work Week!) No Nights/No Weekends Minimal Call (Phone Only): SHARED AMONGST PROVIDERS Ideal Candidate: Passion for chronic care management Team player who is interested in being part of a collegial environment focused on efficiency and quality for our adult patient population Ability to work independently and manage various aspects of electronic documentation during the patient care process With more than 1,100 Primary Care Physicians and APPs across our system, we are dedicated to transforming the delivery of healthcare while providing superior patient experience.
