Physician - Community Living Center Director

The Minneapolis VA Health Care System is seeking a Board Certified/Board Eligible physician to work as the Medical Director of the Community Living Center (CLC).
Responsibilities include medical direction of the CLC program, supervision of provider staff Physicians and Nurse Practitioners, and leadership related to quality improvement projects and accreditation programs.
The Minneapolis CLC is an 80 bed program, consisting of 3 units providing various levels of transitional and long-term care.
There is a specialized secured memory care unit within the CLC with a 10 bed capacity.
The main source of referrals to the program is internal to the medical center, with residents having an average length of stay between 30 and 40 days.
The CLC is staffed by an interdisciplinary team consisting of provider staff (MD, NP), occupational therapy, nursing, physical therapy, psychology, speech and language therapy, and social work.
In addition to CLC, the incumbent will serve as a liaison to the provider staff at the affiliated State Veterans Homes.
The Incumbent will also partner with an inpatient Hospice and Palliative care program located in a portion of the CLC.
The Minneapolis CLC program participates in training of internal medicine residents, hospice and palliative care fellows and other trainees as assigned.
