Overview: The Case Manager serves as a trusted advisor, advocate, and accountability partner for participants by monitoring progress and helping participants navigate through obstacles. Case Managers facilitate both individual and group sessions with participants and serve as the designated leader of participants' care teams. The Case Manager collaborates with the care team to ensure participants are obtaining quality support as they advance through the program model.

Your role will involve: Facilitating excellent participant programming: Conducting impactful 1:1 and group sessions, developing a comprehensive understanding of every participant on caseload and ensuring treatment goals are met Facilitating group sessions focused on life skills such as money management, healthy sexuality, hygiene, self-care, and refusal skills Upholding the quality and fidelity of Concordance Re-entry Model curriculum when delivering 1:1 sessions and groups Delivering participant Reflections when necessary, in a timely and trauma-informed manner Connecting participants to community resources Communicating with multi-disciplinary care team: Documenting sessions (for individual and groups), including noteworthy participant interactions in electronic record system Documenting Reflections using standard forms Working collaboratively with the Community and Life Skills team, career coach, and therapist assigned to each participant, providing input on progress to ensure consistency in instruction/learning pre-to-post release and alignment with participants' goals Participating in internal meetings (Community and Life Skills team meetings, All Team meetings, 1:1 supervisor meetings, and training sessions) Skills, Knowledge, and ExpertiseThe right candidate will come with: Passion for our mission Energy for multi-disciplinary, team environment Excellent interpersonal skills as evidenced by compassion, energy, and ability to build rapport High quality verbal and written communication skills Experience with documenting participant interactions in case notes Familiarity with and endorsement of 12-step programming Required education, licensure, and experience: Bachelor's degree in social service related field (social work, psychology, counseling, criminal justice, sociology, or similar) 1-3 years of experience implementing case management services and documenting interactions Prior experience with justice-involved adults or other vulnerable populations CADC certificate, preferred CPR / Adult First Aid certification preferred What to expect when you join ConcordanceA driven team with a desire to help our participants move forward.
