Job Summary
Position is responsible for the establishment and ongoing management of relationships with Memorial Hermann Physician Network ("MHMD") physicians and their office staff regarding Memorial Hermann resources.
This position works with MHMD physicians to achieve targets set forth in established physician incentive programs and to receive recognition and/or designation from national accrediting bodies (NCQA, Bridges to Excellence, etc.
), insurance companies or other respective entities for accomplishments in disease management, population management, medical cost management and adoption of PCMH principles.
This will involve coordinating and monitoring application processes, tracking and reporting adherence to established benchmarks and observing and making process redesign recommendations to achieve higher compliance and performance.Job Description Minimum Qualifications Education: Bachelors degree in related field or four (4) years related experience

A license in one of the following is preferred:
National Committee for Quality Insurance (NCQA)
Certified Professional in Healthcare Quality (CPHQ)

Experience / Knowledge / Skills: Three (3) years of healthcare experience with an understanding of physician needs, practice patterns and workflow Strong communication and presentation skills required Interpersonal skills necessary in order to interact effectively and build strong relationships with physicians, staff and all levels of management Excellent organizational and prioritization skills; ability to lead multiple projects at the same time and obtain desired outcomes on schedule Analytical skills necessary to identify customer needs in order to develop action plans Working knowledge of Microsoft Office - Excel, Word, and Power Point Working knowledge of major quality improvement efforts (PQRI, NCQA, CI, HEDIS, etc.)
Ability to drive to MHMD meetings and MHMD physician offices in regional area Principal Accountabilities Serves as primary point of contact between MHMD and MHMD physicians and their office staff regarding the implementation and usage of Memorial Hermann resources deployed to advance PCMH and ACO initiatives.
Accountable for deployment of programs at the practice level, including but not limited to, introducing and educating practices on the value and use of reporting tools, patient registries and delivery of reports via our physician portal for enhanced patient care.
Facilitates regular, consistent improvement meetings to monitor, present, and discuss progress on action plans and achievement in milestones.
Maintains the requisite skills to complete analysis on data and processes in order to recognize and analyze undesirable patterns or trends in utilization or performance and obtains the needed resources to develop strategies for redesign and intervention to demonstrate improvement.
Consults and partners with regional medical directors, management, and the practice to identify organizational and structural challenges hindering achievement of desired program outcomes.
Documents action plans and details of visits and outcomes.
Reports critical incidents and information regarding quality of care issues.
Assists in investigating and resolving managed care reimbursement issues identified by providers and/or payers, including root cause issues including fee schedule and reimbursement loading of contracts.
Travels throughout regional area to meet with MHMD physicians and office staff; Attends MHMD office meetings and conducts presentations.
Ensures that documentation produced and/or processed complies with state regulations and/or accrediting body requirements.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
