HOURS: 40 hours per week, no weekends, no holidays
Monday, Wednesday, Thursday 7:30am-4:30pm; Tuesday 9:00am-6:00pm; Friday 7:30am-2:00pm
We service the community by partnering with local employers to provide employees access to our network of primary care practices. We are modeled after Concierge Medicine and go above and beyond to provide the highest level of customer service and care to our patients. The member's experience at HealthWorks is extremely convenient and accessible, with our locations being onsite or near site to the employer, and well dispersed throughout the county. HealthWorks providers and clinical support staff provide relationship-based care and are able to spend plenty of time with members. Our network of practices is growing and we currently have 9 locations.
POSITION SUMMARY: Provides patient care under the direction of the Registered Nurse and within the auspices of the licensure. Patient care includes care to patients in the following age groups: adolescent (13 through 18 years), adult (19 through 64 years) and geriatric (65 years and above).
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Gathers relevant patient information pertinent for the nursing assessment and assists the RN in developing and modifying the plan of care.Organizes and implements designated nursing interventions and basic patient hygiene and comfort measures.Assists physicians with implementing the medical treatment regime.Evaluates and reports to the RN the status of, and changes in the patient's condition.Documents pertinent information and nursing actions and outcomes in the medical record. SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above: Assists in managing emergency patient situations following established protocols and under the guidance of the RN.Assists the RN in implementing patient/significant others' teaching plans.Participates in the on-call responsibilities for this position as required.Other duties as assigned. Provides and performs all administrative responsibilities (complete and accurate patient registration including but not limited to walk-in, check in, and/or scheduled appointments); obtain accurate insurance cardPrepares patient for visit by using appropriate standard toolsAdminister injections as prescribedProvides clinical advice by phone/electronic correspondence in accordance with established triage protocolsMaintain/update membership file for each company monthly or as neededPerforms POCT including but not limited to flu, urine, strep and COVID testsPerforms Phlebotomy on siteAssures exam rooms are adequately supplied and cleaned after patient visitsAssists in coordination and scheduling of referrals and tests (x-ray, US, MRI, etc.) including but not limited to obtaining test authorizationObtaining medication authorization and pending medication refills for providerOrder and stock supplies, including but not limited to immunizations, office supplies, medications and medical suppliesAssist with patient outreach as neededServes as a patient advocate by interacting with patients, visitor and colleagues. Communicates with all patients, visitors and colleagues in a professional manner with customer service as a top priorityReports to provider patient's status/changes in conditionDocuments pertinent information, nursing actions, and outcomes in medical recordMay travel to various HW sites to support operational needsParticipate in team and nurse huddles as required JOB REQUIREMENTS MINIMUM REQUIRED QUALIFICATIONS: Current licensure as a Licensed Practical Nurse, issued by a program for practical nursing accredited by the PA State Board of NursingCPR Certification required PREFERRED QUALIFICATIONS: One (1) year of hospital/clinical experience Benefits At A Glance:
