Oversee/provide patient-centered comprehensive, primary care services at the Health and Wellness Center including:Performing episodic, preventive, and/or primary health careDeveloping personalized treatment plans in collaboration with patientsConducting physical exams, health-risk assessments, and history reviewsProvide referrals to appropriate medical resource or specialty providers as neededParticipates in the development of annual Health and Wellness Center goals and objectivesProvides clinical leadership to medical assistant, patient advocate (embedded case management), and clinicaldirector to ensure a collaborative, team-oriented environmentSupports, leads, and models the vision, mission, and values of the companySkills and Qualifications:
