Primary Care Physician (PCP) Responsibilities: Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.Coaching for health: The PCP acts as a health coach, helping patients set health goals and working towards them.Simplifying for action: The PCP simplifies and prioritizes appropriately to facilitate behavior change for patients.Independently provides care for patients with acute and chronic illnesses.Engages with the hospitalist for hospitalized patients.Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination.Leads their care team for patients able to come to the office.Engages with the transitional care team for patients unable to come to the office.Takes an active role in the management of their center.Performs other duties as assigned.KNOWLEDGE, SKILLS & ABILITIES: Availability and accessibility for patients.Service orientation towards patient care.Evidence-based medicine knowledge.Willingness to develop leadership skills.Commitment to quality care.Ability to influence patients and teams positively.Self-care awareness.Proficiency in Microsoft Office Suite.Ability and willingness to travel locally and nationally.Fluency in English; additional languages may be required.Independent judgment in patient care.EDUCATION AND EXPERIENCE CRITERIA: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.Must have a current, active State Medical License.Board certification in relevant specialty preferred; Board Eligibility required.Maintain board certification through necessary MOC, CME, or exams.Must have a current DEA number for controlled substances.Basic Life Support (BLS) certification required within the first 90 days of employment.We're ChenMed and we're transforming healthcare for seniors.
