As the Continuing Care Community of the Archdiocese of New York, we see enhancing the lives of our elders and others who need extra help to stay healthy and live life to its fullest as more than just a job. To us, it's a privilege and our calling. ArchCare Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for seniors who need nursing home level care but want to stay at home. With PACE, participants have a dedicated Care Team to coordinate everything, so they are able to live comfortably where they are, knowing they'll get the coordinated care they need. PACE provides and coordinates care at our Wellness Centers, in patients' homes, and in other settings.

Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants with a full comprehensive face to face exam including patients institutionalized.Provides evaluation and treatment of participants with acute illnesses, and works with other staff to ensure patient is safe and healthy.Attends IDT's and morning report within your area.Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.Documents changes in the participant's condition and details care provided by completing all required documentation.Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.Attends and participates in scheduled staff meetings and participant care meetings as requested.Participates in meetings for PCP's such as QI, IC and UM, and updates on CMS rulings.Communicates with consultants/Specialists regarding patient care.Responsible for keeping our EMR inputting progress notes and reviewing consultation recommendations, up to date on a daily basis.Participates in on-call duties, on call is Friday to Friday x 7 days every 8 weeks.Job Type: Full Time
QualificationsQualifications: 1 year of post-graduate experience in Internal or Family MedicineBoard Certified in Internal Medicine or Family PracticeEducation: MD or DO DegreeCompletion of residency in Internal or Family MedicineGeriatric experience preferredBilingual preferredSalarySalary: $240,719.93 per year
