PRIMARY CARE PHYSICIAN (PCP) RESPONSIBILITIES: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.The PCP acts as a health coach, helping patients set health goals and frequently following up on those goals.The PCP simplifies and prioritizes appropriately to facilitate behavior change for patients.The PCP must demonstrate the ability to function both independently and in collaboration with other health care professionals.The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Independently provides care for patients with acute and chronic illnesses.Takes full accountability for patient care and outcomes and seeks consultation from specialists when needed.Engages with the hospitalist for patients admitted to the hospital.Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation.Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator.Engages with transitional care team for patients unable to come to the office.Participates in management of their center and helps cover for other providers as needed.Performs other duties as assigned.KNOWLEDGE, SKILLS & ABILITIES: Availability and accessibility for patients to build trust.Service orientation, providing care that they would want for a family member.Stays updated on evidence-based medicine and lifestyle factors impacting patient health.Willing to develop and improve leadership skills.Commitment to achieving high-quality outcomes and continuous improvement.Proficient in Microsoft Office Suite and other relevant software.Ability and willingness to travel locally and occasionally regionally/nationally.Fluency in English, with additional language skills as beneficial.Requires independent judgment.EDUCATION AND EXPERIENCE CRITERIA: MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.Must have a current, active State Medical License.Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred.Must have a current DEA number for schedule II-V controlled substances.Basic Life Support (BLS) certification required within the first 90 days of employment.We're ChenMed and we're transforming healthcare for seniors and changing America's healthcare for the better.
