Program Director - Blended Case ManagementCenClear is hiring! We are seeking a Program Director for our Blended Case Management (BCM) program.
About the Program The BCM program supports adults, adolescents, and children with mental health diagnoses, helping them remain in their communities. Case managers provide access, coordination, and support for necessary services, while offering advocacy and hope.
MissionLead and manage the BCM program, ensuring high-quality service while adhering to the Sanctuary model and regulatory guidelines.
Hours Full-time, 40 hours/weekLocation This position is housed at the behavioral health office located in Saint Marys, PA. Blended Case Management services are offered in Blair, Cameron, Elk, Huntingdon, Juniata and Mifflin Counties. Travel will be required as needed for the position. Compensation for time and mileage occurs based on the reimbursement policy.
