Purpose & Impact:
Incumbent will support the mission and purpose of the Case Management Organization program. Will assist CMO Program Director by providing direction, guidance, and leadership to Case Managers and Case Manager Supervisors.
Essential Functions:
Monitors daily operation of assigned Case Management units to assure client safety and case management progression toward agency's contract goals.
Identifies cases of special interest or increased risk, and arranges for staffing's with Case Manager, Supervisor and Program Director.
Reviews random samples of case files and FSFN notes from each assigned unit to determine effectiveness of services and quality of documentation.
Meets with each assigned Supervisor at least once every month to review cases and to ensure Supervisor's compliance with bi-monthly one-on-one supervision of Case Managers.
Meets regularly with Program Director to review cases.
Participates with Program Director and Quality Assurance in formulation of agency policies, and with agency CQI activities.
Attends staffing's and meetings as required.
Is available by cell phone 24 hours 7 days per week unless on agency approved leave.
Performs written employee evaluations for all assigned Case Manager Supervisors.
Responds to staff and clients to ameliorate problem in a timely and professional manner.
Utilizes critical thinking and sound judgement to problem solve with case management teams.
Serves as back up to Program Director when necessary.
Participates in recruitment, hiring, support, and training of new employees.
Other Functions:
Performs other duties as may be required to meet the goals of this program.
Physical Requirements:
Must have a high level of energy, be adaptable to irregular hours, be able to be on-call as needed, and be available to travel as required.
Education:
Bachelor's degree in Human Services; Master's degree preferred.
Experience:
Must have a minimum of 5 years of job-related experience and 3 years of supervisory experience.
Skills:
Must possess demonstrated leadership and administrative abilities, be able to interpret statistics in order to monitor contract performance, be able to establish effective working relationships with agency staff, families, caregivers, and with outside service providers, and be effective at communicating at all levels.
Other:
Must demonstrate sensitivity to our services population's cultural and socioeconomic characteristics and needs.
Principal Accountabilities:
Reports to Program Director.
Team player with co-workers, administrators and funders.
Accurate, complete, and timely submission of required statistical reports.
Adherence to all licensing and quality assurance standards.
Adherence to agency policies and procedures.
