Psychiatric Nurse Practitioner - Part TimeWe are seeking a part time Psychiatric Nurse Practitioner 8-12 hour per week as part of Mohawk Opportunities' Assertive Community Treatment (ACT) Team: This unique team is an evidence-based treatment program that utilizes a community focused, multidisciplinary team of staff to deliver flexible, comprehensive treatment and rehabilitation services to individuals residing in the community with a severe mental illness.
The program's goal is to help individuals achieve stability within their community.
Services are intensive and non-traditional in nature and provided by a team in the community including in the client's home.
Outreach is a key component of the service model.
The Psychiatric Nurse Practitioner is a member of the community based multidisciplinary treatment team, and in conjunction with the Director of ACT, provides oversight of the delivery of clinical services.
The Nurse Practitioner has three main functions: 1) provision of psychiatric assessment and treatment utilizing a client centered approach while taking into consideration complex medical issues; 2) sharing knowledge of psychiatric illness and treatment, with the goal of educating and guiding the multidisciplinary team; and 3) oversight of the development, maintenance, and supervision of medication administration.
Specific duties include: Upon admission conduct person centered psychiatric assessments, including but not limited to, psychiatric history, course of illness, response to treatment, mental status examination, and DSM diagnoses utilizing the most recent guidelines.
Collaborate with nursing staff regarding client medical needs and recommendations for referrals to community physicians for further assessment and treatment.
Collaborate with medical providers as needed.
This is a community-based position which includes meeting with clients in their homes, community settings or office.
Prescribe psychiatric medications, provide medication education, assess efficacy of medications, and monitor for side effects.
Attend morning staff meetings as assigned, to review client progress and make recommendations about course of treatment.
Prescribe psychotropic medications and conduct regular assessments of therapeutic responses and side effects and educate clients regarding psychotropic medications.
With other team members, provide support, education, and counseling to family members of clients in order to help them understand mental illness and how to best support the recovery of the client, as needed and if indicated.
Actively collaborate with inpatient providers during the acute or long-term hospitalization of a client.
Maintain active and regular communication and consultation with attending Psychiatrist to facilitate a seamless transition back to community-based services.
Participate in and supervise the development of the comprehensive treatment plan for each patient at treatment planning meetings.
Direct and collaborate with the registered nurses to develop, revise, maintain, and supervise team psychopharmacologic and medical treatment as well as medications policies and procedures.
RequirementsSkills/Qualifications for the Psychiatric Nurse Practitioner: NYS Licensed Psychiatric Nurse Practitioner.
Board certification preferred.
Has strong clinical skills and experience providing treatment to persons with serious mental illness; a minimum of three years of experience in psychiatry is required.
Must have a strong commitment to the rights and ability of each person with a mental illness to make own decisions and choose, live in the community, work, and have access to continuous support and services to enhance wellness.
Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.
Must uphold clients' rights, personal preferences in treatment and treat everyone with respect and dignity.
Continuously acts as a guest in client and family homes, is courteous, polite and an active listener.
Valid and insurable driver's licenses.
Must use your own vehicle for your own transport.
