The QIDP/CTH II Coordinator is responsible for residential facilities programs and staffing. This position manages the development, supervision, implementation, and monitoring of each individual's ATP to ensure that each individual receives intensive and effective training in the areas specified in the single service plan. They also ensure Program Standards are met as regulated by DDSN and Burton Center policies and standards and plans for a program to meet individuals needs 24 hours a day, 7 days a week.
This position requires a Bachelor's degree in the Human Services field and a minimum of 1 year experience working with individuals with disabilities and special needs. They must be certified as a Qualified Intellectual Disability Specialist and have a valid SC Driver's License. Computer skill, oral and written communication skills, and the ability to lift and transport individuals is required. Supervisory experience is preferred.
Duties and Responsibilities:
Supervises and monitors staff. Maintains records for each community training home. Monitors client and home by reviewing progress notes monthly. Develops, implements, monitors, and evaluates intensive training programs designed to: Increase skills and levels of independent functioning of the individuals receiving services. Increase appropriate behaviors. Reduce the frequency of inappropriate behaviors. Insures in-service training is provided for all staff in conjunction with individuals' behavior support plan and implementation. Attends behavior support committee meetings as needed. Consults with psychologist/behavior specialist as needed. Assure all ICF/MR, CTH II and SLPII programs are following licensing standards including in servicing staff. Assure ICF/MR, CTH II and SLP II monthly reports, fire equipment checklists, service notes, ATPs and MARs are completed and monitored. Assure annual inspections for ICF/MR, CTH II and SLP II (Fire Marshall, Sanitation, Fire evacuation, HVAC) are completed. Schedules and conducts individuals initial team meeting, annual reviews, and staffings. Writes the initial habilitation plan and subsequent reviews. Insures the written plan is distributed to appropriate staff. Updates individual's single service plan and residential Support Plans for CTH II and SLP II as changes occur. Insures progress is recorded as needed and at least monthly by service providers. Compiles data and progress reports and completes periodic narrative review describing the individuals progress toward established objectives and other pertinent information impacting on the quality of life and adequacy of service. Conducts 90-day psychotropic team reviews of individuals' psychotropic drug regime and consults with the pharmacist, behavior specialist, physician, and nurse to develop drug reduction programs to advocate medications are eliminated or maintained at minimal levels. Provides background documentation for single plans. Actively and aggressively pursues individuals' placement in the least restrictive environment based on the person's program needs and program team recommendations to include any referrals, presentations, and follow-up. Maintains individuals' records in conjunction with residential staff. Completed required paperwork regarding new admissions and transfers in a timely manner. Attend and conduct residential support plan meetings for ICF/MR, CTH II and SLP II individuals annually Attend PDR meeting for ICF/MR, CTH II and SLP II individuals Directs, supervises, and monitors each individual's program plan. Assures that appropriate training programs are established and implemented for each person daily. Coordinates each individual's activity schedule to insure treatment and each individual 's needs. Monitors behavior support plans and training programs and restraint procedures through periodic observation for all individuals receiving services. Complete annual HRC's for ICF/MR, CTH II and SLP II BSPs annually (target dates) for ICF/MR, CTH II and SLP II (including staff in servicing) ATP revisions for ICF/MR, CTH II, and SLP II Monthly progress notes including ATP monitoring for ICF/MR, CTH II, and SLP II Review and sign off on all BIRs for CTH II, SLP II programs Oversees supervision of staff responsible for the daily direct care and training of individuals. Provides input to the residential supervisor regarding staff performance in individual training activities. Monitors to insure that staff interactions with individuals are appropriate and that each individual's dignity and right to privacy are maintained in program delivery. Reviews day book entries, professional logs, and other data to insure follow-up is carried out. Attends meetings as determined by the Operations Director. Responds to facility emergencies as needed and directed. 18.Monitors all purchasing and inventory regularly to insure supervisor maintains adequate control of all expenditures and accountability of inventory, as designated to the supervisor. Responsible for total operation of the facility in accordance with ICF/MR federal regulations and Burton Center policies and procedures. Monitors staffing patterns of the facility to insure coverage requirements are met at all times. (Supervisor is directly responsible.) Administers individual's medication as necessary. Transports individual's as necessary. Assists with house inventory. Assures daily activities are carried out. Assures individuals are appropriately dressed at all times. Monitors monthly documentation of individual's personal property. Shops for or arranges trips with individual's personal property. Shops for or arranges trips with individuals when clothing is needed. Ability to relate to co-workers and personnel on a working and personal level. Works closely with Case Management.
