Job Summary Under the supervision of the Quality Coordinator and reporting to the Assistant Area Administrator, the Quality Program Analyst for Care at Home is responsible for managing quality and performance improvement initiatives.
This role involves participating in risk management, identifying trends in patient care, and ensuring compliance with Quality Improvement standards.
The Analyst will assist in designing and implementing solutions for quality management challenges and will integrate various data reporting systems to generate statistical reports that highlight variations in clinical practices.
Essential Responsibilities Engage in risk management and identify patterns in patient care issues.
Support the development of policies and procedures aimed at minimizing organizational risks.
Monitor compliance with Quality Improvement standards.
Collaborate on the design and execution of solutions for quality management issues.
Manage ongoing quality and performance improvement projects under the Quality Coordinator's guidance.
Integrate multiple data reporting systems to create and maintain statistical reports for clinicians and leadership.
Secure consensus for any necessary report revisions or terminations.
Coordinate data collection processes and collaborate with data partners.
Contribute to the development of quality control and improvement indicators, establishing data sources and analysis methods.
Ensure team adherence to scheduled activities and reporting deadlines.
Identify trends through multidisciplinary reviews.
Prepare and assist in presenting data during multidisciplinary reviews, team meetings, and quality improvement projects.
Work closely with leadership to compile clinician-specific data, supporting recognition and coaching for performance enhancement.
Conduct concurrent and retrospective reviews in collaboration with the Quality Coordinator, using approved indicators.
Ensure quality improvement activities comply with federal, state, and local regulations.
Help develop quality improvement indicators that align with organizational guidelines.
Perform other duties as assigned.
Experience Basic Qualifications: Minimum of two (2) years of experience in direct patient care, clinical support, or quality improvement.
Education High School Diploma or General Education Development (GED) required.
Licensure/Certification LVN, ART, or RRA certification required.
Additional Requirements Strong communication, consultative, and management skills, with good judgment and self-motivation.
Proficient critical thinking abilities.
Commitment to self-development and effective collaboration, including sharing resources and seeking feedback.
Extensive knowledge of TJC, Title 22, NCQA, CMA standards, TQM, and tools for indicator development.
Familiarity with medical care practices and experience in medical records and chart reviews.
Strong statistical and data management skills.
Ability to analyze complex clinical situations and summarize findings.
Proficiency in relevant software applications.
Ability to work effectively within a Labor/Management Partnership environment.
Preferred Qualifications Experience in Home Care, Home Health, Palliative Care, or Hospice.
Knowledge of performance improvement methodologies (e.g., PDSA, Six Sigma, Lean).
Familiarity with Electronic Medical Records; Epic experience is a plus.
Experience in a large healthcare system.
Certified Medical Staff Coordinator certification preferred.
Medical Terminology certification.
Bachelors or Masters degree in data analytics or a health-related field preferred
