The Health Coaching Manager will manage a remote team of 12-19 RN Case Managers. The team will provide case management service either home-based or in a clinical setting. The HC manager role is designed to be a community position as it can require clinic, hospital, or member home visits while monitoring staff dispersed across the state.
Hybrid Schedule: As field schedule permits - in the office Monday and Friday 9am - 12pm.
\n Essential functionsTrack key performance indicators (KPIs) for the team and ensure goals are met.  Prepare reports for upper management on the effectiveness and progress of coaching programs.Develop and implement training programs to enhance coaching effectiveness and performance. Conduct regular performance reviews and provide feedback for continuous improvement. Foster a collaborative and positive work environment that encourages growth and professional development. Intensive quality monitoring is required. The member population is Medicaid members with chronic conditions or at risk of developing.The position requires weekly and monthly reporting of team engagements, disengagements, standards compliance with documentation, assessments, and follow-ups with members.Manage day-to-day operations of the assigned team and participate as a member of the contract leadership team role and responsibilities.Coaches, guides and directs team members to achieve state goals for improving health outcomes and reducing ER utilization and admission.Ensure contract requirements and deliverables and met and submitted as required.Develops and maintains relationships with key stakeholders. RequirementsCurrent unencumbered RN License in Oklahoma.Four-year degree in nursing, healthcare, business, public health, or a related field.5 years of experience in a healthcare environment including care management and/or healthcare quality improvement.3-5 years managing projects and/or professional staff.Demonstrated experience working in a fast-paced and deadline-driven environment.The position requires knowledge of health and wellness trends and best practices, the ability to work with diverse client populations, and strong organizational and time management skills.Will require proficiency in working within Telligens proprietary data platform Qualitrac and Johns Hopkins population health analytics software, Care Analyzer. Knowledge of and compliance with NCQA and URAC standards is required.Medicaid contracting expertise preferred.Proficient in Microsoft products.Strong work ethic.Innovative.Quality assurance and data analytics.Ability to establish positive relationships with stakeholders both external and internal.Confidence and skill in pitching, presenting, reporting, and recruitment efforts with hospitals, clinics, providers, and community stakeholders.Self-driven and motivated.
