Position Summary:

This integral position with the Community Resources for Stability (CRS) team provides vital services in housing stability, financial assistance, and case management for program participants. Utilizes a Trauma Informed approach to communicate effectively with clients who have multiple barriers to housing. Must excel at recruiting, engaging, and cultivating strong community partnerships while also building good relationships with clients. This role requires outstanding organizational and multi-tasking skills, as you will be working with clients off-site, while also being required to fulfill case management data entry and attend meetings on-site at JFS.

Responsibilities:

Housing Navigation
Locate affordable, appropriate, and desirable housing for participants.
Attend relevant community partner meetings with MDHI, HOST, Salvation Army, and OneHome Case Conferencing meetings and follow all guidelines outlined in the Housing First Model and Progressive Engagement principles.
Engage with and recruit landlords and maintain positive landlord relationships.
Verify literal homeless status, following HUD guidelines.
Ability to drive agency vehicles to help clients attend their housing appointments if needed,
Conduct housing habitability inspections.
Mediate any issues between landlords and program participants.
Maintain positive relationships with the landlords and assist with monthly financial assistance requests and processes.
Provide clients with information and material regarding landlord/tenant rights and responsibilities.
Help clients maintain stable housing by connecting them with other resources, educating them about tenant responsibilities, and connecting them with employment and benefits resources, etc.
Assess clients' barriers to housing and help alleviate them.

Colorado residency is required.

Hiring Range

$28.00 - 30.00 per hour

Case Management
Provide counseling, crisis intervention, and supportive services to assist clients in reaching stability and treatment goals.
Maintain accurate case records; document client contacts and progress to ensure accountability.
Adhere to all grant eligibility and documentation requirements.
Request financial approval for services of the program based on assessment of need and availability of resources.

Program Oversight Onboarding
Act as a point of contact for staff questions and concerns relating to the Rapid Rehousing program.
Coordinate training and onboard new staff on the Rapid Rehousing process and guidelines.
Provide on-going training of new and existing staff to ensure compliance with HUD programs.
Research and educate staff on best practice models for case management.
Assist with interviewing and hiring potential department staff.

Supervision
Act as lead worker over assigned housing navigators/case managers
Maintain availability and provide necessary support/supervision/intervention for crisis situations on an on-going basis.
Serve as the point of contact for housing navigator's clients when the staff are out of the office
Input referrals into OneHome system and assign clients to housing navigators.
Communicate and collaborate with department case managers/housing navigators on status of client goals, progress, and any barriers.
Review and process Paylocity, Company Mileage, and Mid First Bank entries.
Conduct ongoing performance reviews for direct reports on RRH team.
Authorize program services based on assessment of need, grant criteria, and availability of resources.
Understand and adhere to Rapid Re-Housing rules and written standards for RRH from grant funders.

Quality Assurance
Responsible for regular and proactive review of program case files for monitoring progress in preparation of internal and external audits.
Conduct onsite compliance monitoring/quality assurance, identify and communicate compliance issues, and ensure policies and procedures are implemented effectively.
Work with team manager, department Director to ensure that policies are complete and up to date.
Assist with basic data analysis of program effectiveness and impact relative to outcome objectives and measures.
Research, monitor, and properly interpret HUD rules and regulations for various housing programs.?

Community & Department Liaison
Network and maintain working relationships with community resources and partners.
Serve as a liaison for the department with outside organizations on an on-going basis.
Represent the agency at community partner meetings and participate in such work groups/committees as funders may require.
Collaborate effectively across the organization and serve as a liaison on special projects
Respond to information requests from staff, external contacts, and organizations

Fiscal Management
Complete or provide input to reports documenting program activities.
Assist with producing timely data reports, including outcome measures and quarterly reports for contracts.
Track and submit appropriate documentation, billing, and reports to Director.
Enters fiscal and program data into databases and verifies accuracy for check requests.
Completes initial submission of check requests for the RRH team.

Miscellaneous Duties
Input client information into databases as required, and enter detailed case notes in each, including HMIS.
Maintain accurate case records; document client contacts and progress to ensure accountability.
Participate in staff training and meetings as required.
Maintain confidentiality of client and agency information.
Set and uphold healthy boundaries with clients.
Keep regular and predictable attendance.

Requirements

Qualifications:

Education:
Bachelors' degree in social work or closely related human service degree or comparable navigation services experience preferred.

Experience:

2-4 years experience in the social work or human service field preferred.

Other:
Demonstrates strong communication, organizational, and interpersonal communication skills.
Comfortable working with landlords while confidently advocating for program participants.
Knowledgeable about working with diverse groups of people, who have multiple barriers to stability, and how to effectively communicate and problem solve together.
Ability to interact professionally as a team member and independently with agency staff, community members, and a diverse client base.
Ability to multi-task in a fast-paced environment.
Must have excellent computer, data entry, and organization skills.
Must have a car and be insured in the state of Colorado to drive clients to housing appointments.
