The Immigrant & Refugee Services Division at Catholic Charities of the Archdiocese of Newark is seeking a Full Time RHP Caseworkers & Caseworker Assistants to support Refugee Support Services program.
The Refugee Health Promotion Case Worker will be responsible for providing health education and support to RSS program clients. Under the supervision of Program Manager, the duties of RHP Case Worker will include but are not limited to:
• Assist with the coordination, development, and implementation of comprehensive Health Promotion and education classes to targeted individuals.
• Educate clients to enhance their familiarity with and access to the US healthcare system.
• Develop and implement strategies to address individual health issues and provide medical and mental health resource navigation and support.
• Advocate for culturally and linguistically appropriate care within the community, creating and distributing educational materials and/or presentations.
• Collaborate with various community health service providers, such as health educators, healthcare professionals, and community organizations.
• Compile and regularly update case notes in client database to provide document services provided.
• Ensure all data recording in Client Track is timely and accurate including client enrollments, services, referrals, and file closures.
• Stay informed about latest research and trends in health promotion.
• Perform other related duties as assigned by the Program Manager.
