Job Brief
The Nutritionist shall be an active participant in the transdisciplinary team and specifically shall be responsible to compile data, complete nutritional assessments, and develop and monitor a nutritional care plan.

Essential Duties and Responsibilities include :
+ Responsible to be able to utilize residents special equipment.
+ Respond appropriately to changes in the individuals functioning and health status.
+ Participates with interdisciplinary team in the development, implementation, and evaluation of the individuals program plan and functions as a nutritional consultant.
+ Assesses residents needs, recommends diet orders and prepares menus via client and staff interviews and observations, and coordination with program staff.
+ Compiles and utilizes dietary and nutrition care data and all other pertinent data to complete annual and quarterly nutritional assessments and develops a plan of care for community clients based on assessments of their nutritional needs, by developing and formulating appropriate individualized goals.
+ Trains and supervises residential staff in the implementation of a nutritional program; provides appropriate guidance and structure to ensure coordination and quality of nutritional services.
+ Provides dietary and nutrition inservices to residence staff and provides on-going food service training to all staff.
+ Actively implements programs designed by the transdisciplinary team by following prescribed methodology.
+ Provides direct implementation of nutritional program.
+ Maintains records and charts for each person by entering required information (goal progress, weight, height, changes in behavior, etc.)
+ Completes and responds to all requests for information.
+ Participates in evaluations and case conferences, including annual and quarterly interdisciplinary evaluation meetings and completes necessary paperwork.
+ Visits on a regular basis to monitor effectiveness of residents menus and diets and their meal preparation and consumption.
+ Monitors residential staff for adherence to written dietary policies and procedures for all food service activities such as food purchasing and receiving, food handling and storage, food preparation and service, and sanitation and safety.
+ Evaluates food served and the methods by which it is serviced for adherence to quality standards and diet orders.
+ Maintains effective written and verbal communications with agency professionals.
+ Interprets and utilizes pertinent current research related to nutritional care.
+ Monitors and evaluates food deliveries.
+ Other duties as assigned.
Requirements:
+ Bachelor's degree in nutrition
+ At least one (1) year of experience preferred.
+ New York State Certification.
+ New York State Driver's License.
+ This position requires that employees are able to stand and or sit for prolonged periods of time.
The employees must also be able to lift and transport individuals and perform all physical aspects of the job.
