Provides professional clinical nursing care, coordinating the delivery of appropriate and timely care for patients, promoting quality and cost-effective health care outcomes.
Provides preventive health care services, chronic disease management, and healthcare education to patients and/or family members.

Collaborate with the medical provider and all interdisciplinary team members to support patient's individualized medical care to maximize patient healthcare outcomes, actively preparing and participating in daily team based pre visit planning.
Proactively assist nursing colleagues outside of assigned teams based on needs of all patients.
Participate in department and clinic team improvement efforts.
Communicate with staff, patients, and other members of the healthcare team in a way that is supportive, constructive, and positive.
Advocate for the patient/client family, fostering patient/family decision making, independence, self-care, growth and development.
Maintain awareness of comfort and safety needs of patient.
Encourage maximum independence and accountability for self-care involving the patient in decision-making and self-management; explain procedures and treatments to patient in a supportive and reassuring manner.
Proactively manage team panel to determine acute and chronic care needs utilizing EHR and population health software; providing guidance, education and support to patients to improve outcomes.
Perform staff nursing including assessment, documentation, education and administering appropriate medications and/or therapies.
Document assessment findings that are consistent including review of body systems, laboratory results, and diagnostic findings.
Review all inpatient and ER admissions daily, documenting in the patient record and coordinate patient care and scheduling follow up care with providers.
Facilitate the ordering of medications according to practice policy and protocols, including maintaining an active AK-PDMP account and monitoring of controlled medication prescriptions.
Administer prescribed medications, treatments, and procedures in accordance with approved nursing techniques within the nursing scope of practice and KANA protocols using appropriate infection control measures; demonstrate knowledge of equipment use; accurately administer age-specific medications and therapeutics.
Responsible for education and outreach for OB patients.
Work closely with the primary care providers to insure comprehensive collaborative care for patients.
Track postpartum care and newborn immunizations.
Ensure immunizations are current for all patients as needed.
Assess emergency situations, initiate action to reduce, correct, or prevent immediate, ongoing, or potential risks to the patient; monitor changes in patient status and unusual occurrences; initiate, manage, and document the nursing procedures and interventions consistent with standards of care and KANA departmental policy.
Maintain a clean and safe working environment including adequate supplies and upkeep of unit and equipment, reporting any defective or missing equipment and safety hazards.
Adhere to infection control policy and procedures.
Maintain annual skills based competencies including safety and infection control requirements.
Impart educational knowledge to develop skills to enhance decision-making and promote understanding by interacting with patients, family, staff, and community.
Stay up to date on all required quality measures and actively participate in quality improvement activities to ensure measures are being captured and met.
Perform duties as assigned in a responsible, respectful and caring manner to promote effective nursing staff relationships.

Graduation from a two or four year registered nurse program.
Two or more years of clinical experience in an ambulatory/outpatient care clinic setting.
Certificates, Licenses, Registrations
License as a Registered Nurse, current BLS and CPR certifications.
